An EHC Plan is a legal document that describes your education, health and social care difficulties. It covers the extra help you need at school to help you learn.
It's first issued after an Education Health Care Needs Assessment or can be updated following an Annual Review of an EHC Plan. The plan has 12 sections.
Section A
This section is all about you, your views, your wishes and future plans.
Section B
Section B covers your Special Educational Needs (SEN).
This will include things you find difficult at school. This could include:
- Your concentration
- Difficulties with reading or writing
- Keeping up with work
- Feeling anxious
- Struggling to express yourself
Section C
Describes any health needs you may have. For example, epilepsy, asthma or allergies.
Section D
This covers the social care needs you have inside and outside your home.
For example, you might have difficulties:
- Getting around your local area or community
- Meeting up with your friends and taking part in activities like the cinema, swimming or arcades
Section E
This section details the outcomes and targets you'll hopefully achieve with all the support described in sections F, G and H.
An outcome or target could be:
- You'll be able to travel independently to an activity
- You'll be able to attend school
Section F
Describes all of the help you'll receive to support you with the difficulties listed in Section B. This could include:
- Reading and writing interventions
- Weekly sessions with a counsellor or speech and language therapist
Section G
This section covers all of the help you'll receive to support your health needs listed in Section C.
This might be medication, or regular appointments with your asthma or epilepsy nurse.
Section H1 and H2
Describes the help that the council will give you inside or outside your home, so you can live independently.
For example, travel training or a Personal Assistant (PA) who can help you find activities to take part in outside your home.
Section I
Details the name of the school you'll attend.
This section will be left blank in a draft EHC Plan, so you can think about which one you'd like to attend. You'll only see the name of a school in the final EHC Plan.
Section J
Section J covers personal budgets. You can request a personal budget to arrange the support outlined in your EHC Plan to be delivered by someone else.
The council will decide if they can give you a personal budget.
Section K
A list of all the reports that were used to write your EHC Plan.
This will include:
- Reports from your school
- Your views and your parent's views
- The views of professionals who you've met and worked with
